Wellness Health Access Team
A Community Action Team is a group of individuals
dedicated to solving a community problem
using a systems based approach
.

Purpose:
To study the health care system and coordinate activities to increase access to health and
wellness services in Catron County.

Methods:
Coordinate with the Catron County Health Council, the Catron County Medical Center, the Department of Health and other agencies to propose and carry out activities to overcome barriers to receiving health and wellness services.

Improve availability of transportation.
Improve communication about health and wellness.
Increase the size and number of health care
facilities in Catron County.
Increase enrollment in Medicare, Medicaid, SCHIP,
and private insurance.
Support the Public Health Office.
Improve health care awareness.
Increase access to medicines.
Increase access to home health, hospice, etc.
